To get a referral to an external therapist covered by your Kaiser plan:

Step One:

Call the local mental health office of Kaiser Northern California. Ask for an external referral to a Carelon Mental Health Provider. You may have to schedule an appointment with a Kaiser therapist in order to get permission to see an external provider. If they approve, they will give you an authorization number. You must have an Authorization Number to proceed.

Step Two:

Once you have an authorization number to see a Carelon provider, you must call Carelon to have your authorization number linked to Shine a Light Counseling Center. You may also need to request the following supervising provider:

Once you have an Authorization Number assigned to Tim or Maggie, you can fill out the form below to request an appointment.

Your Authorization Letter will look like this. Please be sure it authorizes you to see a Carelon Behavioral Health provider. Note the start date and Authorization Number. We will need these to be able to bill for your sessions.
